Efflux transporters at the blood-brain barrier, such as P-glycoprotein (P-gp), transport potential toxic substances from the brain to the blood and thereby serve to protect the brain. However dysfunctioning of these efflux transporters may occur. In the case of P-gp upregulation, this may lead to pharmacoresistance. In contrast, decreased P-gp functioning may lead to central neurotoxicity. A first step towards understanding more about these mechanisms was to ‘image’ P-gp. Femke Froklage and colleagues decided to tackle this challenge. Will P-gp let itself be imaged? What were the obstacles? And did they succeed in what they were aiming at?
